After the accident
At about 2:30 in the afternoon, the cast iron dust removal workshop was casted at the end of the chain of the cast iron machine chain (6 templates on the chain belt were removed from the gap in the chain belt), and the residual iron was cleaned on the lower slide plate of the chain belt. During the process, due to the hot weather, the helmet was removed from operation. When the worker Ryu on the shift dropped the chain plate to the tail platform, the chain rebounded and Ren Fusheng, who was cleaning up the residual iron, was being cleaned. At that time, the blood flow continued, and the worker immediately followed. After taking Ren Fusheng to the hospital, he was diagnosed with minor injuries (three needles on the head).

Accident analysis
1. Ren Ming’s safety awareness during the course of his work was poor. He did not wear protective labor protection equipment. Wearing a safety helmet was the main reason for the accident.
2. In the course of working on duty, Liu lost the chain plate to the platform at the end of the machine. The chain plate rebounded and was injured. Liu Hongjun did not confirm the safety during the operation. This is the main reason for the accident.
3. The inadequacy of inspection and supervision on the wearing of labor protection products by employees within the workshop is an indirect cause of the accident.

Accident lessons
1. Strengthen employees' safety education, enhance self-protection awareness, and labor protection products must be standardized and worn.
2. Strengthen the safety supervision, strictly implement the safety confirmation system before the operation, and do a good job of safety protection measures against possible insecurity.
3. Organize employees to learn safe operation procedures, safety production confirmation system, safety production joint guarantee system, workshop internal safety training, and enhance employees' safety awareness.
